The basic process involves making an incision of about 1cm in an inconspicuous area under the chin (hidden incision), suctioning subcutaneous fat, and pulling and tying the muscles on both sides. Depending on the choice, the step of making an incision along the creases from the front of the ear to the back, pulling up to the SMAS layer to lift the jawline and neckline, and removing and suturing sagging skin can be added. If only the skin and SMAS layer are pulled without removing submental fat, the effect may be minimal, and in particular, deep neck wrinkles are likely not to improve. When a double chin is already accompanied by sagging skin, it is difficult to achieve your expected improvement with contouring injections alone. This is because after fat cells are broken down, the remaining sagging skin may actually become more pronounced. In cases where both submental fat and sagging skin along the jawline are present, we may recommend double chin muscle plication surgery after a consultation. This surgery aims to improve lower facial wrinkles and double chin by tightening both the sagging skin and stretched muscles together. Results may vary depending on the individual.

The approach for neck wrinkles can vary depending on whether they are horizontal or vertical lines. For horizontal neck lines formed where the skin folds, we consider hyaluronic acid fillers specifically designed for neck wrinkles; for vertical neck bands that become prominent due to excessive contraction of the platysma muscle, Botox is considered. However, when wrinkles deepen with aging and fat accumulates under the chin, fillers or Skin Boosters alone may not be sufficient, so we consult regarding surgical options such as neck liposuction, double chin muscle plication, or a neck lift. In particular, deep neck wrinkles cannot be resolved by a neck lift alone, requiring a process of tightening after submental liposuction.

In addition to sagging chin fat, double chin fat, and neck wrinkles, sagging cheek tissue, prominent buccal fat, and marionette lines also fall within the scope addressed in this surgery. By adding a lift that pulls up and anchors the SMAS layer, we aim to simultaneously improve cheek sagging, nasolabial folds, jawline, and neck wrinkles. However, because marionette lines involve tissues tethered by retaining ligaments as well as localized volume deficits that are difficult to resolve through lifting alone, we can combine the procedure with fillers for hollow areas or Botox to the mentalis and depressor anguli oris muscles if necessary. The basic incision is a hidden incision of about 1cm placed under the chin where it is barely visible, and if a neck lift is performed together, the incision is made along the wrinkles from the front of the ear to the back. After surgery, please apply ice packs frequently to reduce swelling and pain, and it is recommended to apply warm compresses starting from 4~5 days after surgery. Keeping your head elevated above your heart when sleeping helps the swelling subside more quickly.
